Planned Parenthood

HIV/AIDS and their families.

Case Management Services provide a link
between HIV-positive individuals and medical and
social services provided by PPCEP and other
community providers. Case managers complete a
psychosocial assessment for each eligible person
to identify his or her needs. Case managers
coordinate the client’s overall social service
delivery plan through client-centered education
and assessments. Case managers work closely
with clients to access services through Social
Security, Medicaid and other entitlement
programs.
Services for
Underserved Emerging Populations (UEP)
provides intensive case management and
nutritional services to West Texas women living
with HIV/AIDS. Case managers meet with clients
through home visits, office visits, and by
telephone. Additionally, the staff conducts
educational and nutritional workshops to help
increase clients’ skills in regard to
problem-solving, communicating with partner(s)
or potential partners, and to increase their
level of knowledge regarding their nutritional
intake of food items and supplements. Clients
are encouraged to incorporate healthier eating
habits into their lifestyles and improve their
overall quality of life through monthly
educational workshops developed specifically to
help meet their needs.
Early Access Services is designed to educate, empower and
assist individuals who are newly diagnosed or
living with HIV/AIDS, or have medication
adherence problems or nutritional deficiencies
associated with the HIV disease. Specialized
case management services are offered aimed at
helping clients maintain optimal health and
quality of life. Through education sessions,
clients learn about medical, primary care,
treatment information, understanding HIV/AIDS,
nutrition, health and wellness, human sexuality
and legal matters. Clients also receive
